Eligibility for Care and Support

A new national eligibility criteria for adult care and support needs and carer support needs has been introduced as a result of the Care Act:

  • all councils across England must now use the same national level of care and support needs to assess what help they can give you
  • this new national eligibility criteria was introduced on 1 April 2015
  • it focuses on the best outcomes for you and if you decide to move to another area, councils will work together to ensure that there is no gap in your care 

After carrying out a needs assessment, Borough of Poole must determine whether you are eligible for care and support.

You will have eligible needs if you meet all of the following:

Adult care and support needs

  • you have care and support needs as a result of a physical or mental condition
  • because of those needs, you cannot achieve two or more specified outcomes
  • as a result, there is a significant impact on your wellbeing 

The outcomes include dressing, maintaining personal relationships, and working.

Carer support needs:

  • you have care and support needs as a result of providing necessary care for an adult
  • because of providing care your physical or mental health is at risk or at risk of deteriorating or you cannot achieve any of the specified outcomes 
  • as a result, there is a significant impact on your wellbeing 

The outcomes include providing care to other persons, maintaining personal relationships and working.

If you have eligible needs, and would like the council to help you meet these, we will agree with you how this will be done, and set it out in a care and support plan. In all cases, whether your needs are eligible for support or not, we will give you advice and information about what support is available in the community.

The above is a Borough of Poole document from pre 1 April 2019 (unless otherwise stated). It has been adopted by BCP Council and will, in time, be replaced by a BCP Council document.

This document applies to service users from the Poole locality and to staff carrying out functions that relate to these individuals. Minor changes may have been made to ensure contact information and website links are correct and relevant from 1 April 2019.

All policies will now be reviewed as part of a planned schedule, following the Borough’s transition to BCP Council on 1 April 2019.
