Legally, a death must be registered within five days of it happening - although this period may be extended in certain circumstances, for example, when the Coroner has been consulted.
If you are unsure about the time limitations, or think you may not be able to register within five days, please contact the Register Office for further information.
If the death occurred in the borough of Poole it must be registered at Poole Register Office.
If you are unable to visit the Register Office in Poole you can go to another Register Office in England and Wales to make a declaration that will be forwarded to the Registrar at Poole.
In this case, any death certificates you require, and the authority allowing the funeral director to proceed with arrangements, will be sent to you by post.
Please remember however that such arrangements rely on many different organisations and services (postal services), will inevitably delay the arrangements you are making for the funeral. If you choose to use this facility please contact the Register Office in advance for further information.

The person who registers a death is usually the closest relative.
If this is not possible then someone who was present at the death, or who is instructing the funeral director may be able to register instead. An executor is not automatically entitled to register a death and further information on persons able to register is available from the Register Office.

When you go to the Register Office you should take:
- Medical Certificate of the Cause of Death - as issued by the doctor who certified the death.
- Coroner's Document - if the doctor has had to inform the Coroner about the death, the Coroner may need to issue another certificate.
- The deceased's NHS Medical Card - if available.
- Any Pension Book, Certificate or Document relating to any pension or benefits that the deceased was receiving from public funds.
- It is also useful if you have the deceased's Birth and Marriage Certificates (if applicable) - although these documents are not essential provided you are able to give the Registrar the correct information.
The Registrar will give you the following free of charge:
- Leaflets about Widow's Benefits / Income Tax for Widows (where appropriate)
- A Certificate of Registration of death Form BD8 - This is for Social Security purposes only. Read the information on the back of the certificate and if any of it applies to you, fill in the certificate and send it or hand it in to your Social Security Office.
- A Certificate for Burial or Cremation Form 9 - This is know as the Green Form and gives permissions for the body to be buried, or for an application for cremation to be made. This should be delivered to the funeral directors so that the funeral can be held.
However, the Coroner may have made available one of the following in place of Form 9 above:
- An Order for Burial Form 101
- A Certificate for Cremation Form E

Parents who suffer a stillbirth or the death of a child can make a priority appointment at the Register Office on 01202 633744 (usually within 24 hours). Or they can ask for a registrar to go to a hospital or hospice and we will guarantee to attend within 48 hours.
We will meet our appointment times as closely as possible. If you are late for your appointment we aim to see you within an hour. If you are early we will see you as soon as possible.
